Breakthrough Producer Questionnaire

This form is to be filled out by the nominated producer. Please answer the following questions regarding your involvement as a producer on the entered film.

In the case that multiple producers have been entered into this category for a single film, each producer must complete this form individually. Other producers on the film who have not been entered for the category do not need to complete the form.

You will be asked to confirm that the contents of this form are approved by another member of your entered film’s team i.e. another producer / director / writer / HoD and provide an email address for them.

In some of the questions you will be asked to describe your level of responsibility for different aspects of the producing process by selecting from a series of dropdown options. In these questions, the levels of responsibility should be understood as follows:

Wholly responsible you were the sole, primary bearer of the producer’s responsibility for this task, with creative and/or financial decision-making responsibility
Jointly responsible you shared primary producer responsibility for this task with one other person, making joint decisions or some of the decisions
Partially responsible you shared the producer’s responsibility for this task with two or more people, making joint decisions or some of the decisions
Contributed to you offered support to the person primarily responsible for this task, contributing to the decision-making process but without bearing ultimate responsibility
Not involved You had no involvement with this task

 

Please consider your responses based on whether you had sole responsibility, joint responsibility or partial responsibility within the parameters of the producer role. Individuals or companies employed by and reporting to the producer(s) should not be considered jointly responsible or partially responsible where the producer bears the ultimate responsibility.
